Community Health Worker Initiative of Maine

The Community Health Worker Initiative of Maine was founded in 2013 in response to the emerging field of CHW. It focuses on building community knowledge of CHWs and their role in the healthcare system.

Louisiana Community Health Outreach Network

The Louisiana Community Health Outreach Network was founded in 2010 by a group of CHWs. It was developed as a space to connect CHWs and allow them to share information and support one another in their profession. The LACHON’s mission is to provide a community of support for CHWs within Louisiana through developmental learning programs for current and future CHWs. 

Indiana Community Health Workers Association

The Indiana Community Health Workers Association aims to promote the CHWs and their role as members of the greater healthcare team. Its mission is to educate and connect CHWs within Indiana, and to serve as an advocate with statewide policy makers for the value of CHWs as healthcare professionals.

Idaho Community Health Worker Advisory Group

The Idaho Community Health Worker Advisory Group is a statewide organization that engages stakeholders in developing a CHW teaching program in Idaho. Its mission is to promote CHWs as a frontline public health worker who is a trusted member of a given population. It supports CHWs connection and access to the greater healthcare system in Idaho.

Florida Community Health Worker Coalition

The Florida Community Health Worker Coalition was founded in 2011 via a grant to increase research into CHW models of care awarded to the Florida Department of Health. Its mission is to promote the advancement of the CHW career and its specific model of care. The FCHWC links 305 CHWs in the state of Florida to address common problems and concerns and offer a network of support within the scope of their work.

Colorado Patient Navigator and Community Health Worker Collaborative

The Colorado Patient Navigator and Community Health Worker Collaborative is a statewide platform for CHWs. It focuses on defining training of both patient navigators and CHWs in an effort to define their specified roles within the healthcare system. It also focuses on developing methods for private and public health insurance reimbursement of CHWs.

California Association of Community Health Workers

The California Association of Community Health Workers is a statewide advocacy group in California. Its mission is to value and promote Community Health Workers and their model of care, and to support career development of CHWs. It aims to increase access to healthcare, reduce healthcare costs, and promote healthy and happy communities through CHWs. 

Arkansas Community Health Worker Association

The Arkansas Community Health Worker Association was founded in 2013 to as a statewide platform in support of CHW. Its mission is to provide continuing education, training, and career advancement for CHWs, as well as advocating for CHWs to receive reliable state funding. The ACHWA has formed alliances with many healthcare providers and clinics to connect CHWs and their patients to care services.

Arizona Community Health Worker Association

The Arizona Community Health Worker Association is a statewide organization founded in 2001. It aims to provide a forum for CHW’s to collaborate on community, governmental, and healthcare-related issues pertaining to their work. Its mission is to lead CHWs through resource sharing, developing partnerships, promoting health and wellness, and strategizing to prevent disease and improve the overall health of Arizona’s citizens.

Alaska Community Health Aide Program

The Alaska Community Health Aide Program is a statewide organization formed to link Alaskan CHWs. It joins together a network of about 550 CHW working in over 170 rural villages of Alaska. These CHW all operate under the Alaska Community Health Aide/Practioner Manual. Its mission is to define CHWs as a profession and connect them into a referral system that of physicians, clinicians, and regional hospitals.
