Strategy 1 lists the protocol developed and implemented for integrating peers into HIV care teams. Provides sample forms as tools for organizations to use when recruiting clients and using peers to engage and retain PLWH in care. Strategy 2 lists the protocol developed and implemented for collecting HIV laboratory data and documenting providers’ data follow-up for the purpose of client surveillance. Provides sample forms as tools for collecting and updating data.

Best Practice Guidelines for Implementing and Evaluating Community Health Worker Programs in Health Care Settings

A report published by Sinai Urban Health Institute. Summarizes CHW professional literature and national and local surveys, successes, lessons learned, and case studies from the field. Aims to address the gaps in knowledge about how to effectively implement the CHW model. Targets health care organizations interested in implementing the CHW model in their system or improving upon their current system. Intended to offer guidance in decision-making and solutions to common program implementation challenges. Topics include: approaches to CHW hiring, training, and performance evaluation; supervision challenges and strategies for success; providing a positive organizational climate to facilitate CHW integration; other effective elements of program design; and program evaluation, including standardized CHW process and outcome measures.

Your Heart, Your Life, A Community Health Worker’s Manual for the Hispanic Community

A manual published by the National Institutes of Health. A heart health manual created specifically for Latino communities for use in training CHW, educating members of Latino communities, and other community activities. Consists of 12 sessions with step-by-step instructions on how to teach 11 educational sessions on preventing heart disease to community residents or patients.

A Community Health Worker Training Resource for Preventing Heart Disease and Stroke

A user-friendly curriculum that CHW can use in their training and also as a resource. Offers plain-language basic information and activities to increase skills in preventing heart disease and stroke. Also covers lifestyle-related issues and risk factors for stroke, heart attack, heart failure, atrial fibrillation, and diabetes.

Best Practices for Integrating Peer Navigators into HIV Models of Care

A report by AIDS United. Presents lessons learned from three AIDS United peer navigation program initiatives: Positive Charge (2010-2014), Retention in Care (2012-2016), and the Access to Care Social Innovation Fund Initiative (2010-2015). Offers ways to successfully and efficiently integrate peer navigators to reach vulnerable HIV populations in America. Topics include: initial steps to begin/expand a peer navigation program (identify target populations, best practices checklist, protocols and procedures), strategies to implement and monitor the program (communication, training, documentation), and ways to address the sustainability of the program.

This five-day curriculum provides training materials and instructions train HIV-positive peers to support patients with care and treatment in your community. It was developed as part of the Peer Re-Engagement Project (PREP) evaluating a program that brings HIV-positive peers into the HIV care team to support patients who have fallen out of care or who are newly diagnosed with HIV and at risk of falling out of care.
