This study uses Electronic Health Records (EHR) to examine the impact of exposure to CHWs on chronic disease indicators among subjects at two different health centers. Though additional EHR analysis is necessary to further assess the impact of CHW intervention, participants in this study showed improved chronic disease indicators after exposure to CHWs.
