An organization dedicated to promoting and sustaining the integration of CHW into Michigan’s health and human service systems through coordinated changes in policy and workforce development. Contains many resources ranging from explanations of CHW role and impact to CHW training and evaluation services to CHW articles and reports.

Massachusetts Association of CHW is a statewide professional organization for CHW from all disciplines that strives to lead the movement to organize, define, and strengthen the profession of community health work. Mission is to strengthen the professional identity of CHW, foster leadership among CHW, and promote the integration of CHW into the health care, public health, and human service workforce.

A manual developed through the International Diabetes Foundation’s Women in India with GDM Strategy program. Educates community health workers about prevention and management of gestational diabetes mellitus.

The Louisiana Community Health Outreach Network was founded in 2010 by a group of CHWs. It was developed as a space to connect CHWs and allow them to share information and support one another in their profession. The LACHON’s mission is to provide a community of support for CHWs within Louisiana through developmental learning programs for current and future CHWs. 

The Kentucky Association of Community Health Workers was formed in 2016 as an advocacy group for the CHW profession. It views CHWs as the missing link between patients and healthcare providers, viewing CHWs as frontline public health professionals. The KACHW’s mission is to unify, promote, and sustain CHWs within the Commonwealth of Kentucky.

The Indiana Community Health Workers Association aims to promote the CHWs and their role as members of the greater healthcare team. Its mission is to educate and connect CHWs within Indiana, and to serve as an advocate with statewide policy makers for the value of CHWs as healthcare professionals.

The Idaho Community Health Worker Advisory Group is a statewide organization that engages stakeholders in developing a CHW teaching program in Idaho. Its mission is to promote CHWs as a frontline public health worker who is a trusted member of a given population. It supports CHWs connection and access to the greater healthcare system in Idaho.

The Florida Community Health Worker Coalition was founded in 2011 via a grant to increase research into CHW models of care awarded to the Florida Department of Health. Its mission is to promote the advancement of the CHW career and its specific model of care. The FCHWC links 305 CHWs in the state of Florida to address common problems and concerns and offer a network of support within the scope of their work.

Kit includes educational materials for CHWs, a flip chart, healthy eyes brochure, information on Medicare eye care benefit eligibility, and more. Available in English and Spanish.
