Sustaining the Family Leader Role focuses on family partnership sustainability within the state teams, strides made thus far, remaining challenges, and possible paths forward. Sustaining family leader engagement in clinical settings and partnership with family-led organizations is about organizational partnerships at its core, and it requires teams to acquire additional financial resources.

This poster discusses a primary care medical home’s efforts to improve family engagement, family well being, child quality of life, and a shared plan of care in CMC. The CoIIN family survey was administered by the clinic’s Family Work Group, led by our Family Liaison. This survey established a baseline for these metrics. The poster was originally presented at the AMCHP 2020 Conference.

The poster discusses a unique intervention between a primary care medical home and inpatient hospitalist team to utilize telemedicine to coordinate care for CMC. This collaboration resulted in reduced readmission rates, though no change in length of stay. This poster was presented at the 2019 AMCHP Conference.

Catastrophic Rupture: A Memoir of Healing was written by K. Jane Lee, MD, a member of Team Wisconsin. As a pediatric critical care physician and an ethicist, Lee was accustomed to caring for children with a range of serious conditions and disabilities, and felt comfortable helping families navigate decision-making for these children. When a complicated delivery left her second child with a severe brain injury, she found that everything she had learned about disability and personhood as a physician and ethicist was no help as a parent.

This book allows the reader to walk alongside the author as she struggles to bond with and love her daughter, as she reconciles what is happening at home with her ongoing role as a physician to patients and families in similar circumstances, and as she shifts from the medical perspective of disability as an impaired body to the mother’s perspective that sees the beauty and value in the person that is her child.

This article, published by AMCHP, features the CMC CoIIN state team journey maps that were created during year 3 of the project. These maps helped state teams visualize their journeys and opportunities for improvement.
