Integrating Community Health Workers Into a Patient-Centered Medical Home to Support Disease Self-Management Among Vietnamese Americans: Lessons Learned

Describes how a patient-centered medical home and academic partners collaborated with CHW to develop and implement the Patient Resource and Education Program (PREP). Under PREP, CHW received training in core competencies related to self-management support principles and conducted a 4-month intervention with Vietnamese American patients dealing with uncontrolled diabetes and/or hypertension.
